Need fertility specialist in-network with EmblemHealth HIP HMOI also worked with Eric Forman (after disappointments with several other practices) and am a huge fan. He is the most responsive, communicative doctor I’ve ever had. And also the reason I have a second child.

Need fertility specialist in-network with EmblemHealth HIP HMODr. Eric Forman at Columbia Fertility is in network with Emblem HIP HMO. He's a great doctor and an amazing human being. I speak from personal experience as he is the reason I have a child. I have also referred him to two other people who are now parents.

ISO fertility doctor who accepts GHI/Emblem HealthI used Dr. Eric Forman at Columbia who was just the best. I had GHI/Emblem at the time and it worked smoothly. I would highly highly recommend him - as would my friends who worked with him based on my recommendation!!

Re: [ANONYMOUS] Advice re. low amh levels?Dr. Eric Forman, RMA NJ. Most highly recommend if can travel to NJ a few times...can do NY monitoring if in a cycle. Extremely accessible and responsive and excellent medically.
