Dr. Eric Forman at Columbia Fertility is in network with Emblem HIP HMO. He's a great doctor and an amazing human being. I speak from personal experience as he is the reason I have a child. I have also referred him to two other people who are now parents.ISO fertility doctor who accepts GHI/Emblem Health

Dr. Eric Forman, RMA NJ. Most highly recommend if can travel to NJ a few times...can do NY monitoring if in a cycle. Extremely accessible and responsive and excellent medically.
