I always recommend Carvel! There is a Carvel on Church Avenue in Kensington. I LOVE their ice cream cakes! You can custom order a size or buy the ones that are already made. You can add a photo on it too but give it a couple of days.

I'll add that you don't have to go all the way to Bay Ridge for Carvel, maker of some of the best ice cream cakes! There's one right off the Church Ave F/G stop. Got a great Fudgey the Whale cake last year... For my own birthday :)
